Dear Parents/Guardians,
FCS is excited about this upcoming school year! We have deployed parent permission forms to all Parent Portal accounts. All forms have important information and need to be reviewed carefully. Please click here for basic instructions about how to sign forms in Parent Portal. See below for a list of the permission forms that need to be completed by August 18, 2026.
All students have the following forms to be completed:
* Parent Permissions,
* Distraction Free Policy,
* Instructional Affirmation Form, and
* Optional Library Book Notification - Only sign if you want notifications from the media center regarding the books your student checks out.
Students taking specific classes or in specific grades have the following forms to be completed:
* Fit Georgia Form - for students taking PE classes in grades 1 - 12,
* Sex Ed Permission - for students taking Health in grades 6 - 12,
* CTAE/Fine Arts Safety Form - for students taking CTAE or Theatre classes in grades 6 -12,
* Chromebook Permission Form - All 6th grade students and new students in grades 7 -12, and
* Scoliosis Denial Form - All 6th and 8th grade student.
Please note only one parent needs to sign each form.

REMINDER: If you have not submitted a copy of your student’s Social Security card to your student's school or at the time of registration, your student will NOT be HOPE eligible.
Social Security Cards may be securely uploaded (click here for the FCS SIS Document Upload Site) or hand delivered to the Data Clerk at your student's current school for processing.
Please contact the Data Clerk at your student's current school of enrollment should you have any questions or need further assistance.
Please disregard this reminder if you have already submitted your student's Social Security card.

Parent Portal Activations - Forsyth County Schools sends an email to parents (to their email address on file) that do not have a Parent Portal account established every Monday. This email provides a web link to set up your account. The link is active for two days. Please check your Spam/Junk folder if you have not received an email.
If you have not received an email, you may contact your child’s school to ensure the correct information is on file. If the correct information is not on file, they will advise you on how to update your information so that you will receive an activation email.

Please go to your child’s school with valid picture identification and they will assist you with a creating a Parent Portal account. Upon login, you will be directed to create a permanent password.
Parent Portal Resets - If you are locked out or cannot remember your Parent Portal username and/or password, please go to your child’s school with valid picture identification and they will assist you with a temporary password. Upon login, you will be directed to create a permanent password.
Student Accounts - Contact the Educational Technology Coach (ETC) at your school for assistance.
